BACKGROUND:
On July 5, 2022, the Mayor and City Council awarded a Price Agreement for the 2022 Electrical Maintenance Assistance Contract, in the amount of $150,000.00, to Van-Mack Electric Inc. Subsequently, the Mayor and City Council have approved Change Order Nos. 1, 2 and 3. The Public Service Committee will review this matter.
CONCLUSION:
Change Order No. 4, a net addition in the amount of $4,006.19, is the result of relocating a traffic signal service at the intersection of Caton Farm Road and Wesmere Parkway due to Com Ed utility relocation and maintenance work in the area.
Sufficient funds exist for Change Order No. 4 utilizing the PW General Fund Electrical - Supplies / Equipment Parts (Org 09028000, Object 536104, $4,006.19).
Also, please process Payment Request No. 9 and Final, in the amount of $4,006.19, on behalf of Van-Mack Electric Inc.

Based on the above, it is recommended that the Mayor and City Council take the following actions:
1. Approve Change Order No. 4, a net addition in the amount of $4,006.19, to the 2022 Electrical Maintenance Assistance Contract.
2. Approve Payment Request No. 9 and Final, in the amount of $4,006.19, on behalf of Van-Mack Electric Inc.
