BACKGROUND:
In advance of the proposed 2024 Streetscape Improvements on Chicago Street, stabilization work will be completed on existing sidewalk vaults to ensure safe pedestrian access to businesses during mainline construction activities. On May 16, 2023, a contract was awarded to complete work at 16 W. Van Buren Street and 79 N. Chicago Street. Change Orders No. 1 thru 4 were previously approved adding the vaults at 56-58, 68-76, and 78 N. Chicago Street. The Public Service Committee will review this matter.
CONCLUSION:
This project has been completed, inspected, and accepted by the Department of Public Works.
Change Order No. 5, a net decrease in the amount of ($294,939.88), is the result of the following items:
1) Elimination of removal and disposal of contaminated material not needed.
2) Reconnection of the roof drain at 78 N. Chicago Street.
3) Balancing of completed items.
Funds will be reimbursed to Capital Funds (Org 30090270, Object, 557200, ($294,939.88)).
Also included is Payment Request No. 6 and Final, in the amount of $76,927.55, on behalf of Lindblad Construction Co. of Joliet Inc.

Based on the above, it is recommended that the Mayor and City Council take the following actions:
1) Approve Change Order No. 5 for the Sidewalk Vault Improvements - 16 W. Van Buren Street & 79 N. Chicago Street, a net reduction in the amount of ($294,939.88).
2) Approve Payment Request No 6 and Final, in the amount of $76,927.55, on behalf of Lindblad Construction Co. of Joliet Inc.
