BACKGROUND:
The City of Joliet Fire Department is often the first point of contact for those in need of treatment. The Will County Health Department is also part of the needed resources for people in need. The City of Joliet and Will County Health Department collaborate to link families to community resources, facilitate partnerships, streamline communication around referrals, and close the referral communication loop for participating organizations.

The City of Joliet wishes to partner with the Will County Health Department in order to utilize an Integrated Referral and Intake System (IRIS). The IRIS System is a web-based referral application that supports warm handoffs and improves communication among partners by streamlining the referral process, closing the communication loop for referring organizations, and giving communities a clear data driven picture of partner capacity. The IRIS allows agencies to refer people to us and we can refer people to other agencies. This is also a way to streamline referring our residents to the Thriveworks Mental Health Program. As the program expands to other communities, using the IRIS System to refer people will help ensure that only Joliet residents fall into the Joliet fund. This system will also allow us to track our data better. Our Community Care Program can also be part of this system. This item was reviewed at the Public Safety Committee Meeting held on March 5, 2024, and recommended to the full Council for approval.

CONCLUSION:
The attached Memorandum of Agreement (MOA) describes the sharing of data within Will County. Partnering Agencies agree to participate in a data partnership with all partnering agencies that sign this Agreement. The attached ...
